Print and fill out by hand
Print both forms and complete them in ink. If you do not have a printer, we can print a copy for you at the event — just let us know ahead of time by texting (772) 877-5787.
Bring them to the first event — or email them ahead
Hand the completed forms to a UGI staff member when you arrive, or email them to hello@uncommongroundinitiative.org before your teen's first event.
One time only
Forms are on file after the first event. You will not need to re-submit unless your information changes.
